Required Donor Information for Contributions Over $50 Full Name: The donor's complete legal name (first and last name, including any middle initial if provided). Mailing Address: The donor's full residential address, including street, city, state, and ZIP code. This is critical for verifying eligibility and reporting. Occupation: The donor's current occupation or job title. Employer Name: The name of the donor's employer or, if self-employed, the name of their business. If unemployed or retired, this should be indicated. Contribution Limits: For Ohio PACs, the individual contribution limit is $16,615.67 per election (primary or general, effective February 25, 2025). Ensure donations don’t exceed this per donor per election cycle. Prohibited Contributions: Verify that the donor is not a prohibited source, such as a foreign national, corporation, or labor union (unless the PAC is structured to accept such funds under specific rules). Anonymous Donations: Ohio law prohibits anonymous contributions over $25. For donations over $50, the above details must be collected to avoid being classified as anonymous. Reporting Requirements: Contributions over $100 must be itemized in the PAC’s campaign finance reports filed with the Ohio Secretary of State. For donations of $200 or more, additional scrutiny may apply to ensure compliance with state and federal overlap rules (if the PAC also operates federally). Provide accurate donor information as required by Ohio law.
